1. Introduction

Reweave Solutions, LLC, an Oklahoma limited liability company doing business as Reweave Health ("Reweave Health," "we," "us," or "our"), respects your privacy. This Privacy Policy describes how we collect, use, share, and protect information about you when you visit reweavehealth.com (the "Site") or interact with us through the Site.

This Privacy Policy applies only to information collected through the Site. It does not apply to information collected, processed, or stored through Reweave Care, our recovery monitoring and care coordination platform. Reweave Care operates under separate privacy and security commitments documented in the agreements between Reweave Solutions, LLC and the clinical organizations that license the platform, and in the Business Associate Agreements and Qualified Service Organization Agreements that govern protected health information.

By accessing or using the Site, you acknowledge that you have read and understood this Privacy Policy. If you do not agree, please do not use the Site.

2. Information We Collect

Information you provide

When you contact us through the Site (for example, by submitting a contact form, requesting a conversation, or sending email to one of our published addresses), we collect the information you choose to provide. This may include your name, email address, organization, role or title, phone number, and the contents of your message.

Information collected automatically

When you visit the Site, certain technical information is collected automatically. This may include your IP address (which may indicate your general geographic region), browser type and version, operating system, the pages you visit on the Site, the time and duration of your visit, the website that referred you, and the type of device you are using.

Cookies and tracking technologies

The Site uses cookies and similar technologies to function and to understand how visitors use the Site. See Section 4 of this Privacy Policy for details, including the categories of cookies in use, your choices, and how to manage your preferences.

3. How We Use Information

We use information collected through the Site for the following purposes:

  • To respond to your inquiries and communicate with you about your interest in Reweave Health.
  • To provide information you request, including follow-up communications about Reweave Care, our partnership posture, or our content.
  • To improve the Site, our content, and our visitor experience.
  • To understand aggregate usage patterns and trends.
  • To prevent fraud, abuse, and security incidents.
  • To comply with legal obligations and to enforce our Terms of Service.

We do not sell your personal information. We do not share your personal information with third parties for their own marketing purposes. We do not use your personal information for automated decision-making that produces legal or similarly significant effects on you.

4. Cookies and Tracking Technologies

The Site uses two categories of cookies and similar technologies. The Site presents a cookie consent banner on your first visit, allowing you to accept or decline non-essential cookies. You can change your preferences at any time using the link in the Site footer.

Strictly necessary cookies

These cookies are essential for the Site to function. They enable basic features such as page navigation, secure connections, form submission, and remembering your cookie preferences. The Site cannot operate without these cookies, and they do not require your consent.

Analytics cookies (Google Analytics 4)

With your consent, the Site uses Google Analytics 4 ("GA4"), a web analytics service provided by Google LLC, to understand how visitors use the Site in aggregate. GA4 uses cookies (for example, _ga and _ga_[ID]) to collect information about your use of the Site, including pages viewed, time spent, and interactions. We have configured GA4 with the following safeguards:

  • IP address anonymization (last octet truncated before storage).
  • Data retention set to 14 months.
  • Google Signals and advertising features disabled.
  • Data Sharing settings minimized in accordance with Google's GA4 admin controls.

GA4 cookies are not loaded until you accept analytics cookies in the Site's cookie consent banner. We use Google Consent Mode v2 to govern when GA4 receives data based on your consent state.

Information collected by GA4 is processed by Google. For more information about Google's privacy practices, see Google's Privacy Policy at policies.google.com/privacy. You can opt out of GA4 across all sites by installing the Google Analytics opt-out browser add-on at tools.google.com/dlpage/gaoptout.

Your cookie choices

You can manage your cookie preferences for the Site by clicking "Cookie Preferences" in the Site footer. You can also control cookies through your browser settings. Most browsers allow you to view, block, or delete cookies. If you block strictly necessary cookies, some features of the Site may not function properly.

5. How We Share Information

We share information collected through the Site only in the following limited circumstances:

Service providers

We use third-party service providers to operate the Site. These include providers for hosting (Google Cloud Platform, including Firebase Hosting), domain and DNS services (Google Cloud), analytics (Google Analytics 4), and email delivery. These providers process information on our behalf under agreements that require them to protect that information and to use it only for the services they provide to us.

Legal compliance

We may disclose information if required by law, by subpoena, or by court order, or if we believe in good faith that disclosure is necessary to protect our rights, the rights and safety of others, or to investigate fraud or security incidents.

Business transfers

If Reweave Solutions, LLC is involved in a merger, acquisition, asset sale, financing, or similar transaction, information may be transferred as part of that transaction. We will notify you of any such change to this Privacy Policy through the Site or by other reasonable means.

6. Your Rights and Choices

Depending on your location, you may have specific rights regarding your personal information. Regardless of your location, you may contact us at any time at privacy@reweavehealth.com to ask what personal information we hold about you, to request correction or deletion, to withdraw consent, to object to processing, or to receive a copy of your information in a portable format. We will respond consistent with applicable law.

Response timelines

We respond to verifiable consumer requests within the timelines required by applicable law. For requests under the California Consumer Privacy Act (as amended by CPRA), we respond within 45 days of receipt, with a single 45-day extension where reasonably necessary and on notice to you. For requests under GDPR or UK GDPR, we respond within one month, with a two-month extension where the request is complex or numerous, on notice to you. Where applicable law specifies a different timeline, we follow that law.

Global Privacy Control and similar opt-out signals

Where required by applicable law, we honor browser-based opt-out preference signals such as Global Privacy Control (GPC). When the Site receives a recognized GPC signal from your browser, we treat it as a request to opt out of the sale or sharing of personal information for the browser and device that sent it. We do not sell personal information and do not share it for cross-context behavioral advertising; the GPC signal is honored on the same posture for the avoidance of doubt.

Do Not Track

Some browsers transmit a Do Not Track (DNT) signal. There is no industry standard for how websites should respond to DNT. The Site does not currently respond to DNT signals. We honor Global Privacy Control (GPC) as described above, and we honor verifiable opt-out requests sent to privacy@reweavehealth.com regardless of browser signals.

California residents (CCPA / CPRA)

If you are a California resident, you have the right to know what personal information we collect, the right to delete personal information we hold about you, the right to correct inaccurate personal information, the right to opt out of the sale or sharing of personal information (we do not sell or share for cross-context behavioral advertising), the right to limit use and disclosure of sensitive personal information (we do not use or disclose sensitive personal information for purposes beyond those expressly permitted), and the right to non-discrimination for exercising these rights.

EU and UK residents (GDPR / UK GDPR)

If you are a resident of the European Union, the European Economic Area, or the United Kingdom, you have the right of access to your personal information, the right to rectification, the right to erasure, the right to restriction of processing, the right to object to processing, the right to data portability, and the right to lodge a complaint with a supervisory authority. The legal bases on which we process personal information through the Site are: your consent (for analytics cookies), our legitimate interests (for security, fraud prevention, and basic Site operation), and our compliance with legal obligations.

Other jurisdictions

Residents of other jurisdictions may have additional rights under applicable local law. Please contact us at privacy@reweavehealth.com to inquire about your specific rights.

7. Children's Privacy

The Site is not directed to children under 13 (or the equivalent minimum age in other jurisdictions). We do not knowingly collect personal information from children. If you believe we have collected information from a child, please contact us at privacy@reweavehealth.com so we can investigate and, if appropriate, delete it.

8. Data Retention

We retain personal information collected through the Site only for as long as necessary to fulfill the purposes for which it was collected, to comply with our legal and regulatory obligations, to resolve disputes, and to enforce our agreements. Specific retention periods depend on the category of information.

  • Contact form submissions and related correspondence. Retained for up to 24 months from the date of last meaningful contact, after which we delete or anonymize the record. We may retain a longer period where you have an active business relationship with us, where you ask us to retain your contact, or where retention is required for legal, regulatory, or audit purposes.
  • Analytics data (Google Analytics 4). Retained for 14 months as described in Section 4, after which Google deletes it under our configured retention policy.
  • Server logs. Retained for up to 90 days for security, fraud prevention, and operational diagnostics, after which logs are deleted or aggregated.
  • Records held under legal hold. Where applicable law, regulatory obligation, litigation, or investigation requires longer retention, we retain only what is necessary to satisfy that obligation, for the period it requires, after which the record is returned to the standard retention period.

You may request earlier deletion of personal information about you at any time by contacting us at privacy@reweavehealth.com. We will honor verifiable deletion requests consistent with applicable law and our retention obligations.

9. Data Security

We use reasonable administrative, technical, and physical safeguards to protect information collected through the Site, including encryption in transit (HTTPS for all traffic to and from the Site), access controls on our administrative systems, and operational practices designed to limit unauthorized access. No system is perfectly secure, however, and we cannot guarantee the security of any information transmitted to or from the Site. Please do not transmit sensitive personal or health information through the Site. For health-related discussions, please contact us at info@reweavehealth.com to arrange an appropriate channel.

10. International Users

The Site is operated from the United States. Information collected through the Site may be transferred to, processed in, and stored in the United States or in other countries where we or our service providers operate. By using the Site, you consent to this transfer and processing. Where required by law (for example, for transfers from the EU or UK), we rely on appropriate transfer mechanisms such as Standard Contractual Clauses with our service providers.

11. Third-Party Links

The Site may contain links to third-party websites or services. We are not responsible for the privacy practices of those websites or services. We encourage you to read the privacy policies of any third-party sites you visit through links on the Site.

12. Changes to This Privacy Policy

We may update this Privacy Policy from time to time. When we do, we will revise the "Last Updated" date at the top of this Privacy Policy. If changes are material, we will provide additional notice (such as a banner on the Site or an email if we have your address). Your continued use of the Site after changes are posted constitutes acceptance of the updated Privacy Policy.
